Apply to Support Our Education Efforts as a Member of the ACP/DMOST Facilitator Team!
Are you passionate about empowering others to make informed healthcare decisions? The Delaware Quality of Life Coalition (DQOLC) ACP/DMOST Program is seeking dedicated facilitators to lead Advance Care Planning (ACP) and Delaware Medical Orders for Scope of Treatment (DMOST) educational sessions across the state.
DQOLC has a vendor contract with the state for this statewide education/training.
Professionals or community leaders with an interest in advance care planning
Excellent communication and presentation skills
Experience engaging diverse audiences—patients, families, healthcare professionals, and community organizations
Comfortable facilitating both in-person and virtual sessions
Willingness to utilize presentation materials (slide decks/handouts) provided through DQOLC
Lead interactive ACP/DMOST education sessions statewide
Engage participants in meaningful conversations about healthcare choices
Present to a variety of audiences, including the public, healthcare workers, and community groups
DQOLC will provide slide deck presentations for use as well as paper handout materials
Help promote informed decisions and quality of life for all Delawareans
Both in-person and virtual session options. Will begin scheduling sessions by mid June. Sessions will be approximately an hour in length.
Daytime, evening, and weekend sessions available
Ongoing support and facilitator resources provided
There will be monetary compensation for travel and time spent
